The Live Well | Bien Vivre Health Coaches have the training and knowledge to assist you in taking responsibility for your own wellness, providing the support, tools and information you need to achieve your wellness goals.

Whether you need support with diet and exercise, to stop smoking, to improve your self-confidence or help navigate the healthcare system, our Health Coaches can help. They will meet with you in a community location that’s convenient for you, or by phone, and assist you in making real changes that will impact your health.

A Health Coach will:

  • Work with you on a one-on-one basis to assess your lifestyle;  to help you set attainable goals and create an action plan that meets your individual needs;
  • Track and monitor your progress;
  • Guide, motivate and connect you with available community resources to help you achieve your wellness goals;
  • Increase your confidence to make the lifestyle changes you desire; NOT prescribe, diagnose, recommend or provide specific health-related information.
