Overview

The Cancer Patient Navigation Program is a community-oriented service with a goal to provide assistance and support to patients and their families throughout the cancer care continuum, especially in the pre-diagnostic and post-treatment transition to cancer survivorship periods. This service is available through Horizon Health Network and Vitalité Health Network.

Eligibility

This service is free and available through Horizon Health Network and Vitalité Health Network. You can be referred by any health care professional (doctor, nurse practitioner, social worker etc.).

Description

Having a diagnosis of cancer turns you and your family’s world upside down overnight. You may have questions about test results, treatments or about trying to secure financial resources for travel and medications. The Cancer Patient Navigation Program can help you and your family with these issues. Cancer Patient Navigation is a process where a person with a suspicious finding or known cancer is guided through each step in the cancer care journey.
